Common Types of Pigmentation in Singapore

At first glance, many pigmentation conditions look very similar. Melasma, freckles, sun spots, acne marks, and Hori’s Naevus can all appear as dark patches on the skin. The differences between them are often subtle, and it can be challenging to distinguish one from another without proper medical assessment. This is why it is common to feel unsure about which treatment is most suitable.


It is also possible to have more than one type of pigmentation at the same time, such as melasma together with deeper pigment or post-acne marks. When this happens, treatment needs to be carefully planned. If the underlying causes are not properly identified, treatment may only address part of the problem. Some areas may improve, while others remain. Over time, repeated sun exposure, skin irritation, or unsuitable treatments can make pigmentation more persistent.

MELASMA


Melasma is a chronic pigmentation condition that appears as symmetrical brown or grey-brown patches, most commonly on the cheeks, forehead, nose and upper lip. It is one of the most common causes of facial pigmentation in Asian skin and may recur over time.


FRECKLES


Freckles are small, flat brown spots that usually appear on sun-exposed skin. Freckles are largely influenced by genetics but become more noticeable with sun exposure. Unlike other pigmentation conditions, they often lighten during periods of reduced UV exposure.


BIRTHMARKS


A common type of discolouration that can be categorized into vascular (red) or pigmentation (brown) birthmarks. Red coloured birthmarks are caused by a localized area of abnormal blood vessels. Brown birthmarks arise from localized areas of excessive pigmentation.

BROWN SPOTS


Brown spots are well-defined patches of pigmentation that commonly develop on areas frequently exposed to the sun, such as the face, hands, shoulders and chest. Unlike melasma, they are usually localized areas of pigmentation rather than a widespread pigment disorder.


POST INFLAMMATORY HYPERPIGMENTATION (PIH)


PIH is skin discolouration of the skin that remains after a wound heals such as acne, cuts, burns. It is the skin’s natural response to inflammation when the body produces more pigmentation during the healing process.

HORI’S NAEVUS


Hori’s Naevus is a benign form of dermal pigmentation that appears as bluish-grey or grey-brown patches, typically on both cheeks. It occurs when pigment-producing cells are located deeper within the dermis and is often mistaken for melasma.

SEBORRHEIC KERATOSIS

Laser Treatment for Pigmentation

Laser treatment is one of the effective options for pigmentation treatment and pigmentation removal, helping to reduce excess melanin responsible for dark spots, melasma, sun spots, freckles, age spots, and post-inflammatory hyperpigmentation (PIH).


While Pico laser is a well-known option for pigmentation, different laser wavelengths and technologies interact with pigment at different depths and have different treatment objectives. As a result, no single laser is suitable for every pigmentation condition.

Pico laser delivers ultra-short pulses of energy to shatter excess melanin into smaller particles, allowing the body to gradually clear the pigment. The different wavelengths target superficial and deeper pigmentation while also supporting overall skin rejuvenation. 

Erbium: YAG Laser  |  2940nm

Erbium: YAG laser gently resurfaces the outermost layers of the skin by removing damaged skin cells and promoting skin renewal. It may improve superficial pigmentation while enhancing overall skin texture and radiance.


Nd: YAG Laser |  1064nm / 532nm

A versatile laser commonly used in pigmentation treatment. Depending on the treatment settings, it can target excess pigment, tiny blood vessels beneath the skin, or deeper skin layers to stimulate collagen remodelling and support healthier skin.

Yellow Laser  |  577nm

Yellow laser selectively targets both pigment and abnormal blood vessels. It may be included as part of a personalised treatment plan for pigmentation conditions where increased blood vessel activity is thought to contribute to persistent pigmentation, particularly in melasma.

Is Laser the Only Way to Treat Pigmentation?

Many patients begin with pigmentation creams and skin-lightening products. While these may help with mild or superficial pigmentation, they often have limited effect on deeper or more stubborn pigmentation. When topical treatments are insufficient, laser treatment for pigmentation is commonly considered as the next step.

Modern laser technologies, including picosecond (Pico) lasers, are highly effective at targeting excess pigment. However, pigmentation is often influenced by more than melanin alone. Underlying inflammation, abnormal blood vessels and skin barrier changed. This is where RF microneedling can complement laser treatment by targeting factors that lasers cannot directly address.

Melasma involves more than excess pigment. Beneath the surface, overactive melanocytes, chronic inflammation, abnormal blood vessels, and a weakened basement membrane all contribute to persistent pigmentation.

Ultra-fine microneedles deliver RF energy directly into the dermis, this stimulates the skin's natural repair processes while helping to address the underlying skin changes associate with melasma.

How Many sessions to remove pigmentation?

There is no fixed number of sessions required to treat pigmentation, as results vary from person to person. The number of treatments needed depends on factors such as the type of pigmentation, its depth within the skin, underlying triggers, skin type, lifestyle habits, and response to treatment.


Superficial pigmentation may respond more quickly, while deeper or long-standing pigmentation, such as melasma, is usually more challenging and requires a longer-term, customised approach.


In addition, pigmentation conditions—especially melasma—tend to be recurrent. Treatment therefore focuses not only on lightening existing pigment, but also on controlling triggers and maintaining results over time.

Can Pigmentation be permanently removed?

It depends on the type of pigmentation. In some pigmentary conditions, such as freckles and certain sun spots, may respond well to treatment and remain stable with good sun protection. 

Other conditions, particularly melasma, are chronic pigmentation disorders that can recur over time. This is because melasma involves ongoing melanocyte overactivity influenced by many factors such as UV exposure and hormones. 

While melasma cannot be permanently removed, it can be effectively managed. Many patients are able to achieve sustained improvement through consistent long-term management.

Does melasma come back?

Yes. Melasma is a chronic pigmentation condition that may recur, especially with ongoing exposure to sunlight, visible light, heat, hormonal changes or other triggers. Although treatment can improve melasma, long-term management often includes maintenance treatments, medical skincare and daily sun protection.

How many laser sessions will I need to remove my pigmentation?

The number of laser treatment sessions varies depending on the type of pigmentation, its depth, severity and your skin's response to treatment. Some superficial pigmentation may improve within a few sessions, while more complex conditions such as melasma or Hori's Naevus may require a longer treatment plan. 

Does laser treatment hurt?

Most laser treatments for pigmentation are generally well tolerated. Depending on the type of laser used, you may experience a warm or snapping sensation during treatment. Some procedures may also involve numbing cream to improve comfort where appropriate.

How soon will I see results?

The time required to see improvement varies depending on the type of pigmentation, its severity and the treatment recommended. Is Pico laser enough to treat pigmentation?
