What is Cellulite?

​The best approach to treating cellulite, is to first understand the causal factors and the science behind them.

Cellulite takes shape when the fat cells gather and push upwards against the skin even as other connective tissues get pulled downwards. This push – and – pull process results in giving an unsightly, uneven texture to the skin surface. The stomach, hips and thighs are the most common skin areas that cellulite tends to form.

Although cellulite affects both, men and women, it is  the women who suffer more commonly due to the difference in distribution patterns of fat, muscle, and connective tissue. While in men, the fibrous septa holding the subcutaneous fat are arranged in a lattice pattern, they are shaped in a parallel pattern for women.

As we age, these connective tissue septa tend to lose their elasticity and grow shorter. Similarly, the subcutaneous fat begins to accumulate and the skin becomes thinner as we lose collagen. As a result, the fat bulges through to create the unsightly nodules on the skin surface. Circulatory insufficiency and inadequate lymphatic drainage are another reason for the formation of cellulite.

It is a common misconception that cellulite will disappear with weight loss. Unfortunately, that is not necessarily the case always. Since cellulite is located in the upper levels of the skin,  when fatty tissue and volume are reduced, the cellulite does not always disappear. Cellulite condition is difficult to treat as there isn’t any one single treatment modality in the market that can effectively remove all the cellulite. But by using an appropriate combination of medical grade technologies, cellulite deposits can be visibly reduced.

Types of Cellulite

There are three grades of cellulite;

Grade I

The skin becomes pale and when pinched, acquires a sagging tone with an “orange-peel” appearance.

Grade II

Visible lumps and bumps are evident while standing up but disappear when lying down horizontally.

Grade III

The skin exhibits an unmistakably dimpled and wavy appearance, also commonly known as the “mattress” skin tone.

The Process between Fat Freezing & Laser Treatment

The CoolSculpting procedure is primarily applied on patients having more of the ‘pinchable’ type of fat deposits in their bodies. This procedure employs a fat freezing technique to specifically target those very areas that fat deposits exist. The procedure is quite comfortable. A cup-shaped vacuum applicator is placed on the skin surface atop the fatty deposits. Extremely low temperatures of up to -13°C are focused on the target area, to freeze and destroy the fatty cells lurking just below the skin. Over the following 6 to 12 weeks, the targeted fat cells are gradually expelled from the body, leaving you with less localized fat and a better-shaped body.

SculpSure is a Laser Thermolysis procedure that gently heats subcutaneous fatty cells, with the help of laser energy at a specific wavelength. A cooling plate ensures that the skin surface remains unaffected. Initially, the patient’s skin is cooled down and maintained during the course of the laser treatment. What Causes Hair Loss?

The medical term for hair loss is alopecia, a condition that occurs as part of the overall aging process. There are different types and causes of hair loss many of us begin to notice mild thinning of our hair after the age of 40. Hair loss occurs to both male and female, but it is more pronounced in men. Some of the hair loss could be caused by:

  • A strong predisposing genetic factor in accelerated hair loss. 

  • Hormonal changes during puberty, after pregnancy and menopause, and even after taking oral contraceptives may also increase in hair loss.

  • Health conditions, including thyroid disease, iron deficiency anemia, general ill health, and overly strict dieting with protein deprivation can cause more hair loss.

  • Stress can also bring about hormonal changes that are responsible for hair loss. This can be physical stress after a serious illness or recovery from surgery, over exercising and emotional stress.

While most hair loss is not associated with systemic disease, it is important to speak to your physician about the cause of your hair loss and treatment options. 

How Do We Reverse Hair Loss? 

There are a few treatment modalities, we can do to stop hair loss. Hair plant surgery, medications as well as non surgical laser therapy. 

Our treatment for hair loss is to use a combination of energy devices, propriety supplements and sometimes, medication to treat your specific diagnosis. I use a hair reduction laser that is able to generate precise laser energy, using a specific safe wavelength with a particular pulse duration. This specialized laser is designed to penetrate into the skin and be selectively greater absorbed by the melanin in your hair follicles. The heat produced targets only the damaged growth area (“root”) of your hair, and not your skin. This hair reduction laser is therefore a safe modality for all skin types.

All body parts containing hair can be treated. We usually treat the face, upper lip, underarms, arms, back, bikini line and legs. The treatment is very fast and most regions can be completed within 15 minutes, larger areas like the back and legs will require more time to complete. Results from this treatment are seen 2 weeks after the procedure.

Laser hair reduction is typically not painful. However, for greater comfort when treating large and sensitive areas, we apply numbing anaesthetic cream before treatment. No specific after care is required for this treatment, and side effects are very rare and almost never permanent. On the odd occasion, some patients may experience slight redness and swelling after the treatment, but these will subside within a few hours to a day later.

Hair reduction lasers can only treat hair that is in the active growth phase. Because hair goes through three different growth phases, you will need to go through a series of five to eight treatments, generally at four to eight week intervals for long term permanent results. The number of treatment sessions and intervals depends on the area treated. Following this clinical protocol, we can attain permanent reduction in hair growth in up to 80% of patients treated. Our experience is that arm and leg hair are more receptive to be permanently reduced when compared to facial hair. On some occasions, a few hairs may regrow after some time, so a repeat treatment may be needed.

A stretch mark is a type of scar that develops when our skin stretches or shrinks quickly. This abrupt change is usually caused by the ruptured of collagen and elastin, which helps to support our skin. As the skin heals, stretch marks may appear. Hormones and genetic can also affect individual to develop these narrow bands on their skin. Other factors also include:

  • Growth spurts that happen in puberty
  • During Pregnancy
  • Rapid weight loss or gain
  • Rapid muscle growth during weight training

Stretch marks come in different colours such as red, purple, pink, reddish-brown, or dark brown, depending on your skin colour. Early stretch marks may appear as thin red or purple on your skin, it may feel slightly raised and can feel itchy. When stretch marks began to mature, the colour starts to fade and the narrow bands began to sink beneath your skin. You can feel the different texture compared to the surrounding skin layer. Often times, you may also feel a slight depression.

What are the ways to remove stretch marks?

To completely remove stretch marks entirely through the natural way is not possible. However, there are some methods that can help to minimize the appearance of stretch marks and make them fade more quickly.

Other methods we can consider to reduce the appearance of stretch marks are topical creams, medical lasers and other medical devices.

1. Topical Creams
Retinods known as Vitamin A contains retinol formation such as tretinoin that is shown to help increase the production of collagen and elastin fibers which can help to reduce the overall appearance of stretch marks. 

Other formulations such as glycolic acid in combination of ascorbic acid which is Vitamin C is known to also help with reducing the appearance of stretch marks. However, as some of these topical creams may cause side effects, these has to be prescribed by your physician.

2. Medical Lasers 
One of the treatments we use is the fractional Erbium Yag laser. This laser energy is able to deliver gently and generate heat underneath the skin without injuring the skin. It starts the process of using controlled thermal injury to the deeper layers of the skin. It then stimulates production by initiating skin remodeling and skin tightening. One of the reasons why we prefer to use Erbium Yag laser as compared to CO2 lasers is because it produces less downtime and has lower risk of Post Inflammatory Hyperpigmentation (PIH). Patient should expect to have a minimal downtime over the treated area - redness, swelling and flaking over the next few days. 

For immature stretch marks, we use yellow lasers or the long pulsed Nd Yag laser to reduce the appearance of red and purplish stretch marks. This treatment can help to reduce the blood vessels as well as the itch associated with the stretch marks. 

3. RadioFrequency Devices
We approach stretch marks like how we approach scarring. We use small needles to gently puncture your skin and deliver heat energy to the skin without hurting the skin. As the skin heals, the body will began to stimulate collagen production by initiating skin remodeling and skin tightening in the affected area. We have two ablative radiofrequency devices; Puro and Intensif.

The Puro RF treatment stimulates collagen and elastic fiber production by running through an electric current on the affected area. This thermal energy reduces the laxity in the skin by inducing neocollagensis. While Intensif treatment uses an RF micro needling method to emit RF energy through the insulated needles. As the needles are longer, it reaches to the deeper. layer of the skin. Both treatment is suitable for treating stretch marks.

One of the most effective ways to remove it without leaving a scar is by laser removal. I find that the non invasive Q-switched laser is one of the most effective lasers that uses short pulses of energy to remove stubborn tattoos. The short pulse uses a 1064nm wavelength to shatter the tattoo ink particles, breaking it into smaller fragments which can be absorbed by the lymphatic system and be eliminated naturally. Another laser, I like to use is the 755nm Alexandrite Picosecond Laser, a new patented technology that is approved by the FDA. The picosecond (one trillionth of a second) laser uses photo-acoustic energy to zap away the pigments in the tattoos with lesser downtime and side effects. A gentle treatment process that is comfortable and safe for the skin. By targeting the laser on the tattoo ink particles in picoseconds, it can effectively clear stubborn ink colours such as orange, yellow, red, blue , purple and green.

Before the advent of medical laser, common techniques included cryosurgery, dermabrasion or excision were used to remove tattoos. As people (and their tattoos begin to age), the tattoos start to fade and eventually became harder to treat. What is Xanthelasma?

Xanthelasma is a yellowish deposit of cholesterol underneath the skin and it usually occurs on or around the eyelids, and often, develop symmetrically around both eyes. While they are mostly harmless to the skin nor painful, these minor growths may cause the eyelid to droop. It can appear at any age but are more likely to develop during an individual’s middle years – and in women more often than men.

These lesions may remain the same size or grow very slowly over time. It can sometimes join together to form larger lumps. Individuals with xanthelasma may tend to have abnormally high lipid levels in the blood, which is known as dyslipidemia. Lifestyle factors can also cause cholesterol to deposit around the eyes, such as – a diet high in saturated fats and cholesterol, excessive alcohol and insufficient exercise.

What are Nasolabial Folds?

Nasolabial folds are also known as “laugh lines” or “smile lines” are the deep wrinkles or lines that begin at the bottom of the nose and extending down toward the corners of the mouth. Some common factors that causes these lines to start appearing such as age, sun exposure, smoking and facial expression.

As we age, the volume in our cheeks begins to deplete causing the nasolabial folds to deepen as our skin begin to break down the collagen and elastin fibres, reducing our skin’s ability to “snap back” from smiles and other facial expressions. How are Nasolabial folds treated?

Dark Eye Circles – Causes, Treatment & Recommended Removal Method

What Causes Dark Circles Around the Eyes?

Periorbital dark circles are a result of the thin layer of skin below your eyes showing the blood vessels and the blood they contain more clearly than anywhere else on your body. (For reference, this skin around your eyelids, called periorbital skin, is on average about 0.5 mm thick compared to an average of about 2 mm thick on most of the rest of your body.)

Now, as you may or may not already know  the reason, veins often look blue isn’t because the blood inside them is blue; it’s because your skin/subcutaneous tissue only lets blue/violet wavelengths of light pass through it. As a result, only blue light is reflected back and the veins look, well, bluish.

It’s the exact same concept with the skin below your eyes. Those dark bluish circles are (usually) just the result of light being reflected back off of the blood vessels sitting just below the surface of that incredibly thin patch of skin. This is the same reason facial bruises are more prominent below or around the eyes; the thin skin just shows the blood from the ruptured blood vessels a little bit more clearly. So what are the causes for your dark rings? 

  • Stress – When you are stressed more of your blood is directed to your main organs where your body needs it most rather than to your face which is left looking drained and a bit pastier. The contrast between the rest of your skin and the circles beneath your eyes, can make the darkness appear more noticeable.

  • Loss of Collagen – As we age, our skin loses its elasticity and ability to regenerate and as a result, ​ it becomes thinner. This is why more often than not, elderly people will have rather prominent periorbital dark circles regardless of how much they sleep. As with those who are genetically predisposed to having thinner skin below the eyes, it’s just biology.

  • Periorbital Hyperpigmentation – Another thing that can cause dark circles beneath the eyes is something known as periorbital hyperpigmentation, which is basically a condition that results in more melanin being produced by the skin below the eyes, resulting in it appearing to be a darker color.

  • Genetics – Unfortunately, there are some people who are just more prone to develop dark circles. If you have naturally very pale skin in particular then the darker pigmentation and any broken capilleries are more likely to show through.

  • Iron Deficiencies – Dark rings can be a sign of an iron deficiency such as anaemia. For people with less iron in their systems, haemoglobin, the red pigment which carries oxygen to cells, is more easily broken down and can make the skin below the eye appear dark or bruised.

  • Broken Capillaries – The thinnest area of skin around your face is also just underneath the eye. This is also the most sensitive part of your face and the capillaries beneath this thin dermis are often prone to breaking, often through stress or through sun damage. The blood cells which spread out of these spaces tend to gather just underneath the surface of the skin and oxidise and form dark purple hue.

  • Sunken-Looking Eyes – With age, the bones forming the orbit shrink and the orbital cavity gets bigger. We also lose some of the periorbital fat that cushions the eyes. Over time, this can result in sunken-looking eyes and make dark circles look worse.

What Causes Keloid Scar?

Scarring is a natural part of the healing process. Any injury, burn, surgical operation or skin problem, like acne, can cause a scar. Scarring is psychologically distressing and cosmetically challenging to treat. Concealing an inconspicuous scar is easy. However, facial scars or large visible scars are unattractive and can be difficult to conceal with make up.

Keloids are overgrown scars from overly aggressive wound healing. Moles are brown or black colored skin growths on the skin. They occur when melanocytes, the skin cells which make melanin (the pigment to give our skin its natural color) grow in a cluster instead of being evenly spread throughout the skin.

Moles usually appear from early childhood, but sometimes develop during the first 25 years of your life. It is quite normal to have between 5 to 50 moles by the time you become an adult. Moles can be alone or in groups, and can appear anywhere on the skin. With time, some moles can slowly change and become larger, raised or darker in color. Moles can also darken after sun exposure or during pregnancy.

Mole Removal/ Treatment

There are multiple treatment modalities to remove moles, the method of choice is dependent on whether the mole is raised or flat. Raised moles can be removed by an ablative laser - my preferred modalities to use is the Erbrium Yag instead of a CO2 laser for better results. The treatment uses bursts of light radiation to break down the mole cells in the skin. The number of session(s) to remove is dependent on the depth of the mole. 

As for flat moles, ablative laser is not an ideal treatment as this treatment can sometimes leave a depressed scar after the mole is removed. Instead, we use a Pro Yellow 577 nm laser that only targets the pigments for flat pigmented moles.

Apart from excision and cutting methods to remove moles, laser removal is also another option which you may consider if you have multiple moles to remove at the same time or if your mole is located at a harder to reach area.

Our skin produces natural oil in the sebaceous glands, known as sebum. This natural oil is crucial for keeping the skin healthy as it helps to lubricate the skin, preventing it from drying out or become irritated. Fine lines and wrinkles occur because of ageing. As we we get older, the skin gets thinner when collagen and elastin synthesis slow down. This is made worse by UV damage and exposure to environmental contaminants. Wrinkles and fine lines can also be caused by dehydration or allergies in faces of any age. Young children with allergies, asthma or eczema have creases and folds under the eyes, call Dennie-Morgan lines.

Other factors like pollution, nutrition, genetics, smoking, and certain medications can also contribute to the formation of wrinkles. However the majority of wrinkles and fine lines are still related to photo damage from ultraviolet light. Damage from ultraviolet (UV) light exposure is responsible for almost 80% of the skin changes commonly attributed to the aging process. The biggest sources of UV light are the sun and indoor tanning beds.

Ultraviolet light damage causes both a breakdown of collagen and elastin (molecules in the dermis that give structure , strength and elasticity to the skin) by activating enzymes called matrix metalloproteinases (MMP) but also decreases the deposition of new collagen by dermal fibroblasts. When exposed to ultraviolet light, fibroblasts undergo aging or senescence.

Wrinkles and lines first appear in the lines of facial expression. Dynamic wrinkles are the ones that appear when a facial expression, such as a smile or frown, is made and go away when the face is relaxed. Eventually, these dynamic wrinkles become permanent or static as the facial expressions etch them into the skin that has lost some amount of strength and elasticity.

What is Dilated Veins?

Dilated facial veins (Varicose Veins) also known as “spider veins”, is a common benign skin disorder. The widened blood vessels give the appearance of fine, easy visible, threadlike red lines or patterns that appear just beneath the skin surface. It grows gradually and is often seen in clusters. Spider veins are generally seen on areas frequently exposed to sunlight, like the cheeks and the nose. The causes that enlarge these blood vessels are unknown. There is a genetic predisposition, aggravated by sun damage, extreme heat or cold.

Patients more likely to develop spider veins include those who:

  • work or play outdoors;
  • are older, as your blood vessels begin to weaken and spider veins are more likely to form as skin ages;
  • use health and beauty products that cause skin irritation, such as abrasive soaps and sponges;
  • use corticosteroids and some skin products which thins and weakens the skin;
  • drink too much alcohol, which increases the flow of blood in vessels; and are affected with diseases like rosacea, scleroderma, dermatomyositis and systemic lupus erythematosus.

Treatment for Spider Veins

Many patients find their dilated facial veins unattractive and want to have them removed. Treatment is done to improve your skin’s appearance. I treat dilated facial veins using a 577 nm Pro Yellow pulse dye laser. This laser is specially designed to target the widened blood vessel, forcing its walls to collapse. This reduces the appearance of the red marks or patterns on the skin. Widespread and persistent facial flushing which give the face a ‘ruddy’ complexion is caused by many tiny dilated broken facial veins. This condition requires treatment with the Pro Yellow laser scanner which carefully and precisely seals the many dilated blood vessels in the skin.

Treatment with the Pro Yellow laser is painless and takes 5 to 15 minutes. There is no recovery period required and you can continue with your daily activities without interruption. However, to reduce new spider veins from forming, it is good practice to protect your skin from excessive sun exposure and extreme temperatures.

What are Warts? - Causes & Treatments

A skin wart is an infection by viruses in the human papillomavirus (HPV) family. A skin wart usually occur on the forearms, palms, fingers, and/or feet and its appearance depends on its location. A slightly raised small solid growth that resemble a small cauliflower with a rough texture are commonly found on hands. A flat, rough with spongy surface warts are called the plantar warts, they are commonly found on the soles of the feet. Lastly, small slightly raised, smooth warts can be found on the face, hands and shins. They are less common as compared to the other warts and they appear in large numbers. The plantar warts on the feet may be painful due to the pressure areas as they get pushed into the skin. 

Although skin warts are harmless, they are contagious and can spread from one part of the body to another or to children.Thus it is important to always wash your hands regularly. Warts are developed when tiny blood vessels grow into the core of the wart to supply it with blood. To remove it, we must first destroy the blood in the tiny vessels and lasers are one of the ways to do it.

The traditional method to treat skin warts was to use an ablative laser to destroy the wart. However, to carry out a complete removal, the surrounding skin will also get damaged, leaving a depressed scar. The newer method I now practice is to use a vascular laser to cut off the feeding blood vessels to the wart. The infected tissue eventually dies and the warts falls off while the surrounding skin remains intact.
